The Smile For Life Kids Club holds quarterly drawings for prizes for members who have received their cleaning and exam every six months. Once the parent or guardian receives the Smile For Life notice of drawing postcard, they will need to submit the completed dental treatment form, found below, to be entered into the drawing.

If any Smile For Life Kids Club member has not seen a dentist in the last six months, the Dentist Treatment Form can be printed and taken to the next dental appointment and completed.
